HIPAA Notice of Privacy Practices
This Notice describes how Zanason may use and share your medical information and how you can get access to this information. Please read it carefully.
1. Our commitment to your privacy
Zanason understands that your medical information is personal. We are required by law to: maintain the privacy of protected health information we create or receive about you; give you this Notice of our legal duties and privacy practices; follow the terms of this Notice; and notify you if a breach occurs that affects your protected health information.
2. Uses and disclosures without your authorization
For treatment: we share your information with physicians, nurses, and pharmacists who coordinate your care — including our clinical provider Wasef Health PC and the pharmacy that prepares your medication. For payment: we use your information to process your subscription and, where applicable, coordinate with your insurance or HSA/FSA account. For healthcare operations: we use your information to improve quality, train staff, and conduct internal audits. As required by law, for public health, and to prevent serious harm to you or others.
3. Uses that require your written authorization
We will never sell your medical information. These uses always require your signed authorization and may be revoked by you at any time: marketing uses involving your protected information; testimonials or before/after photos that identify you; sharing your information with third parties not involved in your treatment, payment, or operations.
4. Your rights over your medical information
You have the right to: request a copy of your medical record (we will respond within 30 days; a reasonable fee may apply); request a correction if you believe something is wrong; request restrictions on how we use or share your information; request confidential communications to a specific address, phone, or email; receive a list of disclosures we have made of your information; be notified if a breach occurs; and receive a paper copy of this Notice even if you received the electronic version.
5. How to file a complaint
If you believe we have violated your privacy rights, you may file a complaint with us or with the U.S. Department of Health and Human Services. We may change this Notice. If we change it significantly, we will post the new Notice at zanason.com and notify you in your next communication with us. The effective date above indicates the current version.
