Washington Consumer Health Data Notice

This is Zanason's separate Washington Consumer Health Data Notice, required by the My Health, My Data Act (RCW 19.373). It explains what consumer health data we collect on zanason.com, why, who we share it with, and how you can exercise your rights.

Who we are

Zanason Health, Inc. is a Latino-focused GLP-1 telehealth marketing platform. Clinical care, prescriptions, and patient records are provided by our partner Bask Health, Inc. This notice covers data Zanason collects on the marketing site at zanason.com. Bask publishes its own consumer health data notice covering the patient experience.

What we collect

• Quiz responses — your weight, height, goals, and any health conditions you tell us about during pre-qualification
• CTA submissions — your name, email, state when you start signup
• Analytics — pages you visit, buttons you click, device and browser information
• Language preference — your selected language (English or Spanish) so the site stays in your chosen language

We do not knowingly collect data from anyone under 13.

Why we collect it

• Quiz and CTA data are used to pre-qualify you for Bask Health's clinical intake and are forwarded to Bask in real time.
• Analytics help us understand which pages work and which don't.
• Language preference keeps the site in your chosen language.

Who we share it with

• Bask Health, Inc. — our clinical partner. Receives your quiz and CTA data so a Bask clinician can evaluate you.
• Google LLC (Google Analytics 4) — our analytics processor, operating under Google's standard Data Processing Terms and the Google Analytics Data Processing Amendment. Data collection is gated on your explicit cookie-banner opt-in; IP anonymization and ad-personalization signals are disabled at the property level.
• Amazon Web Services — our infrastructure provider, operating under a HIPAA Business Associate Agreement.

We do not sell your consumer health data, and we will not in the future without your separate written authorization.

How long we keep it

• Quiz and CTA data are deleted from Zanason within 24 hours of successful handoff to Bask.
• Analytics data is retained by Google Analytics 4 for the property's configured retention window (default 14 months).
• Bask's retention is governed by Bask's own notice and applicable medical-records law.

Your rights

You can ask us to:

1. Access — tell you what consumer health data we hold about you.
2. Delete — delete it.
3. Withdraw consent — withdraw any consent you previously gave.

Email hola@zanason.com with the subject line WA Consumer Health Data Request. We will respond within 45 days for access requests and 30 days for deletion. There is no fee for the first request in any 12-month period.

Appeals

If we deny your request, you can appeal by replying to our response email. We will issue a final decision within 45 days. If you remain dissatisfied, you may file a complaint with the Washington Attorney General at www.atg.wa.gov/file-complaint.

Changes

We will post any material change here and update the effective date.
